Maximus IX Hero: USB 3.0 Header only working for 1 port?

I have a Maximus IX Hero and a Define R5 which has 4 USB ports for the front panel (2x USB 2.0, 2x USB 3.0). I have both the 20 pin USB 3.0 and USB 2.0 header connected.

When testing my ports, I can get both USB 2.0 ports to work and only 1x USB 3.0 port to work. I first thought it was a broken front panel, but I can confirm both ports worked on my previous motherboard (Gigabyte Z270 Gaming K7). I asked Fractal for a new panel anyway citing the USB 3.0 ports were WAY too tight. I thought maybe I had broken one trying to pry my USB 3.0 stick out.

So in comes the new header and I plug it in and AGAIN only 1 out of 2 USB 3.0 ports work.

I'm scratching my head. Did I get 2 bad panels or is my motherboard misbheaving? I have the Intel Z270 chipset drivers installed--am I missing anything else?

Here's a look at my device manager--does anything look wrong for USB controllers?
