You could also find at what temp your laptop is running. The fans might be clan, but if the thermal paste is dead it makes little to no difference because the chip can't get out any heat.
So, here is what I think you should try (in the given order):
1) see what temps are you running: if very high -> thermal throttling -> you need new thermal paste (given that you said that you cleaned the fans and heat vents)
2) try older drivers, might do the trick
3) maybe a windows 10 clean install (if you have 10 and did the upgrade and never a clean install)

In theory at 86c there should be no throttling, even if your gpu is at 86c. It's a normal side effect of having only one heat spreader for cpu+gpu. But, what are your idle temps (at what room temp)?
As for the Windows, just make sure you have that 1511 fall update. It fixed maybe problems, including the crash after coming back from sleep.

How about you cpu usage on idle? Is it higher than + - 1%? How about ram usage? More than + - 1.8 GB?
One other thing to check for would be viruses/malware. Give it a scan with your antivirus and malware bytes (it's free, so no worries there).
Do you use qBittorrent or other torrent manager? Just yesterday I installed the new version of qBittorrent and when it ran my laptop seemed to behave like my 15 years old pc when I ask it run Eclipse and few applets in the same time. <- unlikely, but worth giving a shot.
The idea is, since when it behaves bad did other changes occur?
