No video on second boot and/or if idles for too long

o video on boot *sometimes* Rampage IV formula and EVGA GTX 680 Signature SC+
My computer will only boot with video on the first boot of the day and only if the power supply has been switched off overnight. If it idles for a long time it will also BSOD and the GPU fan will max out(not an overheating problem i've checked)

When turned on otherwise the following happen:
-my monitor will have a yellow light instead of a blue one
-the graphics card is not detected on my motherboards led(next to PCI bus lane switches)(rampage IV formula)
-the motherboard VGA error LED is on
-the fan spins at 100% then either stops entirely or slows down to normal speed
-on my motherboards debug LEDs the error code AE(Legacy boot event) is displayed or windows boots and the start-up sound can be heard through headset

What I have tried/know so far:
-will boot with video if reset or turned on and off X amount of times
-memory is fine(ran memtest)
-the card has been turned down to GEN 2(possible X79 issue)
-my cpu and ram OCs have nothing to do with this as far as I know
-system can go to 100% load and play intensive games fine without any issues or overheats(100% cpu load temp is 40-46)
-have somewhat checked all cable connections
-have tried re-seating the card
-updated MB bios
-updated GPU drivers

CPU: i7 3820 LGA 2011 @4200mhz
Ram: Corsair Vengence 16gb 4x4gb X.M.P. @1600mhz
Motherboad: Asus Rampage IV Formula
GPU: EVGA GTX 680 Signature Superclocked+
PSU: Corsair 860w 860i

I will try a repair install. Do you think this could be because my mb is slightly tilted? (notcible at rear I/O)

Hmm well,anything that could cause the board to short out has to be looked for; as well as even slight ill fitting, that could cause the GPU to not seat 100% in the slot etc.

I don't like the sound of tilted....have you got a pic?

It looks like the motherboard is 1.5 mm lower than it should be. Could this cause a contact issue?
It should also be noted that my computer works fine when it turns on...

Well, hard to tell....doesn't look too bad but if it is low the concern would be that there is pressure on the card meeting the case? Is the GPU screwed to the case....is it easy to fit the screw if so?

Yeah, couldn't hurt....maybe take the board out and sit it on it's box and connect it up and see if freestanding it has any problems....and then put it back in carefully if not......check the case standoffs are in the correct ATX pattern etc.
