Need help with OC crosshair v formula z and fx8150

Since day 1 of my build I seem to have issues with OC. I posted in the forums awhile back and didn't get to far.
I have since then not oc'd my system at all. I can test all hardware and tests are good. I can wipe my HDD re install everything and still get random freezing and have to force reboot or like as of late i get glitches in os. I have like in background 12 images setup to rotate and they sometimes ghost each other overlapping like both are present at same time,
Also if I run any type of registry cleaning software whole system will lock up causing force reboot
I have made sure all drivers and windows updates are up to date and begin to wonder if hardware was never good from the beginning. My ram I am not sure if fully compatible with the MOBO BUT i seen some say it is.
I really need any help I can get I had my previous build last me 7 years with not half the specs of this system and it still runs to date without any issues.
I am currently running windows 8 64 bit as well. I have Govt issued symantec endpoint protection running and no other crap on it but games and some basic software applications.
Any advice on hardware or how I might need to setup Bios. Please dont refer me to the crosshair v formula z and 8150 OC guide. I simply dont get from it enough to oc my rig. I feel there is something missing or perhaps I am too much of a noob and got lucky on my first build.

Same problem as the op is having in this thread with V readings on the 12v rail.
I have said most what could be said here already, I was hoping HiViz was going to pull something out of his bag of tricks, but I am just as happy if it is you. It may be more beneficial if we try to resolve this in this thread.

My RB2 scores maybe deceiving there is not much competition with the 9series currently and I would expect it to beat most if not all 8350s.
I have tweaked it for sure, and covered almost every setting on my mobo. On my last OCZ when rigged up I assumed it was the PSU, but my Seasonic has been the same from day one so I just assumed that was fine. It was not until HiViz pointed this out that this was not fine I became concerned. Maybe a false reading in Hwmonitor I thought, maybe a 9 series thing. I thought it was my H55 limiting my PC, but now have my custom 360 loop set up and could not get any improvement despite the temps being noticeably lower. So it is not that. Back to either PSU or Mobo, as I am pretty sure I checked a few 8350's and they where 12v+. I have not been able to get a tester for the PSU yet being Christmas madness and all, but I hope to by the end of the week so I can rule the PSU out. I also hope to get a waterblock for the VRMs if they ever come in stock, to try and rule a heating issue out there. I do not have any thermal pads to lift the heatsink and see if there is a problem there or not, but I know mine at least gets far hotter than I am comfortable with. Falling finding a fault with that I am at a loss as I am pretty sure I have not missed a setting trying to sort this out. CPU has been to 1.625, NB 1.9 CPU/NB 1.425 LLC ultra high, plus every combination. What I have noticed is I can not get this to every run in extreme unless I am missing something that is a must to run it in this LLC setting.
I can boot 5.3ghz but no hope completing RB, and 5.4ghz will get to windows log on before it crashes. Mine maybe is the CPU limit, but I am not so sure that is the case for the op.

I think I have covered everything that maybe of some use, please feel free to ask away or try some settings etc. I may have to do screenies tomorrow as my health is not great today.

Ok I did flash bios to latest version verified earlier in thread. This mobo has already been rma'd a few months back. I will take pics of my rig and upload in a min. I will say I put a 700 Watt psu in here delivered same exact voltages as my 1000 Watt. This would mean 2 bad boards in a row(sad face) I will run current requested changes and post back with pics and ty for ya help

The key question is this, what is causing the boards (if it is another bad board) to go bad...

See what I am getting at.

Ok so I did the specs yall requested I set my bios too booted up and now is stuck at Windows screen with no USB power so no power to mouse or keyboard uploading photos please forgive wire mess was nice and neat till I pulled psu out the other day and decided not to put back if I end up rma psu

That is also something I have experinced whilst overclocking. Had to clear CMOS and swap the USB ports to get it to work again. Brand new mouse and keyboard, plus tested and same results with the old keyboard and mouse.

I had to hardboot system down adn reboot to get it to come up and work something is def out of whack seemed like it struggled more and was slower to boot and load.
On another note MY cpu came with the amd water cooler It has 2 wires for fans on back block and connect to cpu heatsink part with one wire u run to top of mobo in cpu fan or opt fan slot and it gives me low cpu fan errror on bootup

Hivizman would love to pull something out the hat I can promise you. But with the time zone making things very difficult for me, tend to spend the evening with family and not on line as a rule I thought it best to rope in my mate and fellow AMD boffin. 😄

OP could you use a fat formatted USB stick and F12 in the relevant bios pages to take screen shots of those settings in bios you have physically changed.
