Need help with OC crosshair v formula z and fx8150

Since day 1 of my build I seem to have issues with OC. I posted in the forums awhile back and didn't get to far.
I have since then not oc'd my system at all. I can test all hardware and tests are good. I can wipe my HDD re install everything and still get random freezing and have to force reboot or like as of late i get glitches in os. I have like in background 12 images setup to rotate and they sometimes ghost each other overlapping like both are present at same time,
Also if I run any type of registry cleaning software whole system will lock up causing force reboot
I have made sure all drivers and windows updates are up to date and begin to wonder if hardware was never good from the beginning. My ram I am not sure if fully compatible with the MOBO BUT i seen some say it is.
I really need any help I can get I had my previous build last me 7 years with not half the specs of this system and it still runs to date without any issues.
I am currently running windows 8 64 bit as well. I have Govt issued symantec endpoint protection running and no other crap on it but games and some basic software applications.
Any advice on hardware or how I might need to setup Bios. Please dont refer me to the crosshair v formula z and 8150 OC guide. I simply dont get from it enough to oc my rig. I feel there is something missing or perhaps I am too much of a noob and got lucky on my first build.

700w is fine for what we are doing stick it in and see if there is any difference.

I will plug the 700 watt in today after work. I have to pull it form another system

So this is my 700 watt PSU at stock and its producing same results.
Now I am beginning to think its not my PSU.
I am going to pop my old one back in and clean it up while waiting on ya response

I am waiting on HiViz to reply as I am not 100% sure.
I may borrow a tester but that will be this time next week at the earliest.
I have a couple of other theories that I would like to rule out, one being the VRM's. Hopefully the water blocks are in stock by this time next week as everywhere I look they are not in stock. Looks like I have to have an EK also.

I have a tester but multimeter so I can do that. But very odd 2 psu showing same thing. And that 1 has been in a over clocked system for 6 years no issues. I feel it's my board or something else simple I'm missing

If you are comfortable with the PSU then all good.

Lets move on.

Can you please do the 22 x 200 settings use VCore of 1.425 for now.

Korvan .. Sorry for your issues M8 .. don't know how I missed this Thread, my apologies. And if I have neglected to
say so before .. Welcome to ROG, Please Pardon my Manners.

Give the above a shot, I would add one other - CPU/NB manual to 1.4 as well ..
Can you take a pic of your rig set up to confirm wiring [please, no offense, just want to help]
Do you have any other monitoring software loaded ? aiSuite ??
Please give the above a go and post a screen of HWmonitor .. and cpuZ x2 .. one of cpu other of memory ..

Your voltages are all over the place .. so as suggested already it can be but two things .. the psu or the board ..
[or some of the board settings .. we will look at that more if needed]
