780 Ti and VG248QE monitor not detected by device manager

I wasn’t sure where to post this since I’m new.

I finished building my rig in June 2014 and haven’t made any changes to its hardware since. I’ve kept all of the drivers and BIOS up to date, and occasionally checked temperatures with HWMonitor, CPU-Z, and GPU-Z. Everything was working fine until yesterday.

I was playing Team Fortress 2 when it started artifacting and froze. I restarted my computer thinking to myself “oh, it’s just another bug Valve should fix.” However, it began to get progressively worse, this time crashing the my computer while the game was loading. I restarted once again, quite annoyed that this was happening. Once I saw the BIOS screen, I knew something was wrong. There was an abundance of white horizontal lines across my screen, which persisted through the entire boot process. When the the login screen loaded, I was even more concerned. The resolution on my 1080p monitor had lowered significantly to 640x480, and the color quality was atrocious. In hindsight, I should’ve checked HWMonitor to see if there was out of the ordinary with the system. Unfortunately I didn’t and can’t provide that information.

I initially suspected it was faulty drivers since 347.52 was released the day before yesterday, and tried downloading 347.25 and trying to reinstall it. After reinstalling and restarting, nothing had changed. I then checked device manager to see if it still detected my monitor and GPU. While the GPU was detected, it had error 43, and the monitor was not listed at all. After attempting to reinstall the latest monitor drivers from Asus’ website, updating the BIOS, and reinstalling 347.52, nothing had changed.

I was hoping that my 780 Ti hadn’t died, so I removed it from the PC and took the cooling bracket off to clean it to see if that made a difference. From the naked eye, nothing looked suspicious and the card did not seem to be fried. I put the card back in to the computer, ensuring it was seated well and booted up again, to find that nothing had changed. I even tried switching from DVI to an HDMI port on the card, which made no difference either.

I even went so far as reinstalling Windows. There was a slight improvement graphics-wise, because when the installer finished and the computer booted up, I had the same 640x480 resolution but with 16 bit color. However, once I had downloaded and installed the Nvidia drivers and rebooted, it reverted to the same color quality as before. It may also be worth noting that I cannot access Nvidia control panel, and that I cannot change the display's resolution or refresh rate from Windows control panel.

I regret not checking GPU temperatures when I first started experiencing the artifacting and crashing, because those would’ve been very beneficial in helping solve this. That being said, here’s screenshots post-reinstallation of Windows:

EDIT: I tried using a different monitor (Philips 19PFL3403D) to see if that would be any different, but it still experiences the horizontal white lines while booting up and has the low color and resolution quality and unchangeable resolution in Windows control panel.

EDIT 2: Solved the problem, apparently the custom cooler I installed was problematic and my reinstalling the default ACX cooler, the card now works.
