USB port numbering

On the Maximus VI Gene ...1) How are the USB ports numbered, top to bottom (↓) or bottom to top (↑) ?2) On page 1-6 of the manual, connection #15 shows USB 1112 and USB1314. How can two USB ports be on one connection ?
