Failing fresh install remove the drivers using DriverFusion (or something similar) in safe mode and reinstall?
Is your RAM rock solid....maybe test with Memtest86+....no problem with any previous GPU? just this new one?
Maybe neaten up the system specs there...it doesn't make great reading:o

Use afterburner or precision X and GPUz to verify what settings you have. Simplu lower core clock and check in GPUz.....lower to standard value for the refference model of the card...removing the factory OC...if the card runs stable you know the factory OC is not all it should be.
